This project defines an application which would create e-forms. All the questions and input types, validations are stored in the databases. This project follows a MVC approach. Please Note that this code requires an extensive database model. To observe main PHP look into the Controller Folder.

Here the project consists of creating pdf forms using Acrobat Reader 9 Pro, and using Submit Button to make an HTTPS submit which calls the PHP Service to store data in the database. This project shows that using FPDM package, we can make confidential PDF forms online without risking privacy. Please Note that this is the test file and because the forms are confidential it has not been available.
